4. <br /> La. R.S. 42:1124.2C(5)(a) requires the name, address, type and amount of each source of <br /> income received by the individual or spouse, or by any business in which the individual or spouse, <br /> either individually or collectively, owns an interest which exceeds ten percent of that business, which <br /> is received from the state or any political subdivision as defined in Article VI of the Constitution of <br /> Louisiana. <br /> 5. <br /> Donald Cravins, Sr. failed to timely and properly disclose income received by himself from <br /> the Louisiana Teachers' Retirement System. <br /> 6. <br /> On September 1, 2010, Donald Cravins, Sr. received a Notice of Delinquency, pursuant to <br /> La. R.S. 42:1124.4, requesting that he amend his 2009 Tier 2 personal financial disclosure statement <br /> to properly disclose income received by him from the Louisiana Teachers' Retirement System. <br /> 7. <br /> Based on the foregoing facts, Donald Cravins, Sr. violated La. R.S. 42:1124.2. <br /> 8. <br /> In accordance with La. R.S. 42:1141C (3)(b)(iv), the Board designates Tracy M. Barker and <br /> Michael D. Dupree as the Board's trial attorneys in this matter. <br /> WHEREFORE, the Louisiana Board of Ethics requests that the Ethics Adjudicatory Board <br /> (a) conduct a hearing on the foregoing charges; (b) determine that Donald Cravins, Sr. has violated <br /> La. R.S. 42:1124.2 with respect to the foregoing charge; and (c) assess an appropriate penalty in <br /> Page 2 of 3 <br />
